- How do I make premium payments?
What are the consequences if I do not make premium payments on time?
If the policyholder fails to pay the premium past the end of the discretionary period, then the policy will be cancelled /lapse, rendering it null and void. The exception is if the policy has a cash value equal to or greater than the amount of the premium arrears, in which case the value of the cash will automatically be used to pay the loan premium or for payment of associated costs in accordance with the type of products owned by the policyholder.
If my policy is cancelled/ lapses, what are the damages, and can I reactivate the insurance coverage?
A cancelled / lapsed policy renders the insurance protection null / void, thus in the case that the insured dies, any Claims will not be paid and no policy benefits will apply. You can reactivate / restore policy a canceled / lapsed policy by completing the form to restore your policy, paying any premiums in arrears, attaching a copy of your valid identity card and enclosing additional documents (health requirements as needed).
When can I apply to restore a cancelled / lapsed policy?
In general, the request to restore a canceled / lapsed policy should take place not more than 2 years from cancellation / lapse of the policy , however this may be adjusted in accordance with the specification of the product owned by the policy holder.

If I lose my policy, can I create a new policy?
Please contact Sequis Care (021-2994 2929) or email to email@example.com in the case of a lost policy. To request a new policy, you can visit the National Service Center or a Regional Service Center. Bring a letter with a duty stamp of Rp. 6,000 stating that you have lost your policy. An administrative fee of Rp. 60,000 will be charged.
