Reactivating/recovering a lapse policy can be made no more than 2 (two) years since the Policy lapsed by filling out the policy policy reinstatement form, completing premium obligation payment by attaching a copy of valid identity and additional documents (health requirements if needed).

The Customer may apply for the Policy creation through the Sequis Service Center at the National Service Center (NSC) in Jakarta, the Regional Service Center (RSC) in Jakarta, Medan, Surabaya, Bandung, Semarang, and Denpasar or the nearest marketing office by carrying the Statement of Losing the Policy from police and pay an administrative fee of Rp 75,000.00 to get a duplicate policy. Policyholder changes can be made at any time as long as there is still an insurable interest element (main family/work relationship) while the Insured changes can not be made / changed because the initial risks and policy benefits have been calculated upfront as stated in the Policy book agreement.

To change information such as data, correspondence address, phone number and email, customers can complete the Policy Change form which can be obtained at www.sequis.co.id and attach a photocopy of the Valid identity and make changes at the National Service Center (NSC) and Regional Service Center (RSC) or nearest marketing office.

To change the data of the Beneficiary, the customer may fill in the Policy Change form that can be obtained at www.sequis.co.id and attach a copy of the valid identity, Family Card / Marriage Certificate / Birth Certificate and make changes at National Service Center (NSC) and Regional Service Center (RSC) or nearest marketing office. In order to change the payment method, the customer can fill out the authorization forms for debit account / credit card, copy of valid identity, photocopy of passbook, copy of ATM card (for account source) and make changes at National Service Center (NSC) and Regional Service Center (RSC ) or the nearest marketing office.

Yes, for fund switching, customers can contact the National Service Center (NSC), the Regional Service Center (RSC) or the nearest Sequis marketing office and fill out the Unit Link Submission form and attach a copy of the valid identity. Fundswitching can also be done through the Sequis Polisku app (available for Android and iOS).

Customer may directly view the list of partner hospitals through mobile apps or at www.sequis.co.id

Customer may choose one of Sequis hospital partners in website and then make the hospitalization process. Customer may contact AdMedika realtime helpline, 7x24 hour online admission at (62-21) 2964 7300 or through IA (International Assistance) at (0603) 1 800 22 7378 (when participants are already in Malaysia) and (065) 3157 9670 (when the participants are already in Singapore).

After the customer register by phone, there will be a verification process. Upon confirmation, the patient can visit the hospital with a membership card for the process, the customer will receive care coverage, deposit guarantee and sign the statement.
