Reactivating/recovering a lapse policy can be made no more than 2 (two) years since the Policy lapsed by filling out the policy policy reinstatement form, completing premium obligation payment by attaching a copy of valid identity and additional documents (health requirements if needed).

Customer may file a request for a copy of Policy through National Service Center (NSC) Jakarta, Regional Service Center (RSC) Jakarta, Medan, Surabaya, Bandung, Semarang, and Denpasar or the nearest marketing offices by submitting the letter of loss from the police and pay an administrative fee of Rp 75,000.00. Policyholder changes can be made at any time as long as there is still an insurable interest element (core family member/work relationship). The Insured name cannot be changed because the initial risks and benefits of the policy have been calculated upfront as stated in the Policy book agreement.

To change Beneficiary data, please fill in the Policy Changes Form which can be downloaded at www.sequis.co.id and attach a copy of valid ID, Family Card/Marriage Certificate/Birth Certificate. In order to change the type of payment, the customer can fill in the authorization form for debit account/credit card, copy of valid identity, a copy of bank statement book, copy of ATM card (for account source). Please visit National Service Center (NSC) and Regional Service Center RSC) or the nearest marketing office to make the changes. Note: For payment modification submission other supporting document needed if a customer has already been registered to the autodebet both account and credit card then to modify the payment, a cancellation form to cancel the previous autodebet account credit card must be submitted.

Yes, for fund switching, customer may contact National Service Center (NSC), Regional Service Center (RSC) or the nearest Sequis marketing office and fill in the Link Unit Submission form and attach a copy of valid ID. Fundswitching can also be done through Sequis Polisku (available via website at https://polisku.sequis.co.id and apps for Android that can be downloaded on PlayStore and iOS in the App Store).

Sure. Minimum Top Up Rp 1,000,000.00 and every Top Up fund is subjected to 5%

Sure, to redirect fund allocation of investment, customer may contact the National Service Center (NSC), Regional Service Center (RSC) or the nearest Sequis marketing office and fill in the Unit Link Submission form and attach a copy of valid ID. Redirection can also be done through Sequis Polisku (available via website https://polisku.sequis.co.id and mobile application that can be downloaded on PlayStore for Android and App Store for iOS).
