• Filing a Death Claim

Procedure of Death Claim Submission:

 

  • The claimant is the beneficiary listed on the policy. 
  • The beneficiary may file a claim under  the condition of the beneficiary's age at the time of filing a claim  is 18 years old. If he/she is still under 18 years old who is deemed to be inadequate by the law, then the claim can automatically be filed by living parents. If the parents have passed away then it is necessary to issue a Letter of Ability issued by the District Court. 
  • If the parents have divorced, then the  one who has the right to claim is the  appointed one to be the Guardian on the Divorce Certificate or Decision Letter from the District Court. 
  • If the beneficiary has passed away and there has never been any change of the beneficiary as long as the Insured is still alive, the person who is eligible to receive the death claims benefit shall be the heir of the beneficiary set forth in the Act.

 

Documents required: 

 

  • Claim Form (Sequis Life Form). 
  • Power of Attorney Letter from beneficiary (family) (Sequislife Form). 
  • Legal Heir Certificate (Sequis Life Form). 
  • Medical Certificate (Sequislife Form). 
  • Copy of the identification papers of the insured, heirs or designated parties that are still valid. 
  • Original policy. 
  • Copy of Family Card (Kartu Keluarga) and Birth Certificate if beneficiary is family. 
  • Death Certificate (original / legalized) or Death Certificate from Local Government Authorities and from the medical authorities (original/legalized).
  • Letter of Assembly Guardianship from the District Court if the heir is not yet or is 18 years old. 
  • Police Statement of Accident from the police in case of accidental death. 
  • If the payment will be authorized to other parties, it only can be authorized to the spouse / parent / child, and must submit a power of attorney on 6.000 stamps equipped with a copy of identity of the grantor and the proxy, accompanied by letter of statement of the branch / agency (Sequis Life Form).

Claims may be filed if the Policy has Premium Free Additional Insurance on the risk of the death active Policy Holder.

 

Procedure of Submitting Premium Free Claims due to Policy Holder dies: 

 

Claim filed by the insured or beneficiary. The substitute of the policyholder is the party who has financial dependency with the Insured. 

 

Documents required: 

 

  • Claim Form (Sequis Life Form). 
  • Power of Attorney Letter from claimant (Sequis Life format). 
  • Description of Heirs Form (Sequis Life Form). 
  • Physician Information Form (Sequis Life Form).
  • Copy of the identity of the Policyholder and Claimant. 
  • Copy of Family Card, Birth Certificate (if the replacement is a child/parent). 
  • Death Certificate (original / legalized) or Certificate of Death from the Local Government and from the medical parties (original/legalized). 
  • Statement of Accident if the death occurred because of accident. 
  • Statement from the Policy Holder who will take over the policy (only allowed a Husband/Wife/Parent/Child). If more than one person is entitled (e.g. the children, or father-mother), all parties must sign the Statement indicating their knowledge and approval of the policy takeover. 
  • Statement of Accident from the police if the death was caused by an accident.
