Policy Reinstatement

Policy reinstatement is the reactivation of the canceled or lapse policies, specific to the type of policy with the facility can be reactivated again. Policy recovery can only be made a maximum of 2 years after the date of the policy are canceled or lapse, after the policy recovery agreement was made through the underwriting process (if necessary).

Policy Recovery Procedures

  1. Fill out and sign the Policy Recovery Policies Form (attached) in accordance with the policy owned
  2. Attach copies of valid ID cardholders (KTP / SIM / Passport), or other supporting documents (if required)
  3. Submit the document to the marketing office / Regional Service Center / National Service Center
